AHCCCS has also communicated that this opportunity has been prolonged until March 2024. Potterville Care assists parents in becoming paid providers for their children, enabling them to offer Habilitation Care Services and Attendant Care Services.
Suppose Habilitation Care Services or Attendant Care Services are not included in the child’s service plan. In that case, your DDD Support Coordinator must assess your child to determine eligibility before adding these services to your child’s planning document.

- Upon finishing your registration, you can visit any of the approved locations to have your fingerprints scanned.

Are there any additional training requirements for Attendant Care?

- Yes, First Aid/CPR and Article 9 training are mandatory for all parent providers. A class schedule will be provided for you to choose a convenient date and time.
